U.S.A. pesticide reporting form
I have posted a pesticide reporting form on my website at this link. CCA is a pesticide application so it is included.

If you have been chemically exposed please report it to ensure that the government is aware by reporting it to all levels of government. While this form is for the U.S.A. I will try and get forms from other countries posted as I receive them. If you have any such forms please forward them for posting.
In the meantime fill out this form and report your exposure to your government and ask for the proper form if you are not from the U.S.A.
