This paper examines the effects of systematic and random errors in recall and of selection bias in case-control studies of mobile phone use and cancer. These sensitivity analyses are based on Monte-Carlo computer simulations and were carried out within the INTERPHONE Study, an international collaborative case-control study in 13 countries. Recall error scenarios simulated plausible values of random and systematic, non-differential and differential recall errors in amount of mobile phone use reported by study subjects. Plausible values for the recall error were obtained from validation studies. Selection bias scenarios assumed varying selection probabilities for cases and controls, mobile phone users, and non-users. Where possible these selection probabilities were based on existing information from non-respondents in INTERPHONE. Simulations used exposure distributions based on existing INTERPHONE data and assumed varying levels of the true risk of brain cancer related to mobile phone use. Results suggest that random recall errors of plausible levels can lead to a large underestimation in the risk of brain cancer associated with mobile phone use. Random errors were found to have larger impact than plausible systematic errors. Differential errors in recall had very little additional impact in the presence of large random errors. Selection bias resulting from underselection of unexposed controls led to J-shaped exposure-response patterns, with risk apparently decreasing at low to moderate exposure levels. The present results, in conjunction with those of the validation studies conducted within the INTERPHONE study, will play an important role in the interpretation of existing and future case-control studies of mobile phone use and cancer risk, including the INTERPHONE study.Journal of Exposure Science and Environmental Epidemiology advance online publication, 14 June 2006; doi:10.1038/sj.jes.7500509.

INTERPHONE

One remark on the Interphone: Choosing the biased definition of regular user in the INTERPHONE is not a "Random error" at all. It is made with an intention. Why don't researchers of tobacco define someone who smokes 1 cigarette a week as "a regular smoker"? Because they are not ready to make a fool of themselves when writing in the conclusions that "regular smokers" didn't have an increased risk for lung cancer. People laugh very loud when I tell them about the definition of the regular user in the INTERPHONE. They immediately get the point and the purpose of the study. I am talking about non- scientific people. It is misleading and not honest to use this definion and then to write at the conclusions (like in the Interphone german study for example) that there was no increased risk for regular users, while the definition itself does not represent regular users at all. Hardell's study is several levels above the INTERPHONE study because Hardell's research is scientific, whereas the INTERPHONE is a political study. After I saw IARC'S Philip Autier saying that dioxin is not carcinogenic for humans while it is on the IARC list of known carcinogens from 1997, I have to think - something is very rotten at IARC and you'd bettter believe it.

WHEN WE THINK OF A TYPICAL 21st century scenario of an urban dwellers lifestyle, we usually include cell phones, PDAs, notebooks, computers, microwave ovens and TVs in the picture. They are very much a part of our world and to many, its impossible to imagine a life without them. And what do all these devices have in common? With prolonged exposure, they can also be a harmful source of electromagnetic field (EMF) radiation.

EMF pollution causes a different type of environmental pollution which can be more risky to ones health than the usual air pollution we can easily detect and protect ourselves from. EMF radiation is a major stressor of the body and we get a fair amount of it every day, everywhere. Yet, we cant see it, we cant hear it, we cant smell it.

Conflicting results

Some researches seem to suggest that the increased incidence of brain tumors during the last decade or so is attributable to cell phone use. Preliminary data would show that the brain tumor is usually located on the side where the patient would use his/her cell phone.

But results from studies have been conflicting. Last year, the Dutch Health Council, reviewing all available data from all over the world, found no conclusive evidence that radiation from mobile phones and TV towers was harmful. A four-year British survey released early this year also showed no link between regular, long-term use of cell phones and brain tumor.

These earlier publications seemed reassuring to cell phone users until researchers at the Swedish National Institute for Working Life observed in their study that the use of mobile phones over a long period of time can raise the risk for brain tumors, contradicting the conclusions of earlier researches.

The Swedish researchers looked at the mobile phone use of 905 people between the age of 20 and 80 who had been diagnosed with a cancerous brain tumor, and they found a link with their cell phone use.

The study noted that 85 of these 905 cases were so-called high users of mobile phones, that is they began early to use mobile and/or wireless telephones and used them a lot. Similar to the observations of previous studies, the tumors were more prevalent on the side of the head where the phone was said to be used.

Increased risk

Dr. Kjell Mild, head investigator of the study, said the figures meant that heavy users of mobile phones, accumulating 2,000 hours or more of cell phone use in their life, had a 240 percent increased risk for a malignant tumor on the side of the head the phone is used. In practical terms, if one uses cell phones an average of one hour a day (total daily use), he would have a 240 percent risk of developing a brain tumor in 5.4 years compared to nonusers of cell phones.

Dr. Mild suggested that to get the risk down, one should use hands-free phones but other experts dont seem to agree. They hypothesize that hands-free devices even serve as an antenna that attract more EMF radiation.

Another important observation in some of these studies is that children and younger people are more vulnerable to EMF waves due to their relatively thinner skulls, which are still maturing and growing. With the unlimited text and call promos available in our country, many youngsters spend minutes and sometimes hours making telebabad on their cell phones. This can pose a potential problem later on.

Inadequate disclosure

Since hands-free devices dont seem to offer any protection, the only safeguard I can think of at this point to minimize EMF radiation from cell phones is to use it only when its necessary. If a landline is available, then we should use it instead of the cell phone.

And just like the cigarette manufacturers, the network providers (Globe, Smart, and Sun) should also have a line at the bottom of their advertisements stating something like: Preliminary evidence suggests that accumulated exposure to cell phone radiation might increase the risk of brain tumors. This should free them from any liability of inadequate disclosure which cell phone users who eventually develop brain tumors might file against them.

Cell phone mast 'affecting my family's health'

(Gulf News Via Thomson Dialog NewsEdge) Dubai: A father fears his family's health is being jeopardised by a cell phone transmitter mast attached to his flat's balcony.

Eappen Thiruvattal, 50, thinks the tiredness, headaches and joint pains suffered by him, his wife and his daughter could be caused by the mast.

Telecommunications company Etisalat insists that its cell phone towers exceed international recommendations and are not a threat to people.

Thiruvattal, an Indian national works as a manager for a video company, grew concerned after learning of the possible health dangers of cell phone masts.

"You read in many places that these can cause brain tumours and other sinister things and now we have one of these masts near us 24 hours a day. It produces microwaves which could be unhealthy."

"My daughter has constant headaches and complains of pains in her knees. My wife is being treated for her shoulder. I suffer fatigue. It's affecting our health," he said.

Thiruvattal has lived in a first floor apartment along Oud Mehta Road with his wife Ruby, 49, and daughter Eva, 15, for nine years.

He said the mast was fixed between his balcony and that of his neighbour more than a year ago.

"I am sure these things are normally fixed on towers so they are at a distance from people. In India, all telephone companies have them on towers," he said.

Cell phone masts have caused controversy across the world, with residents often campaigning for their removal amid health fears.

Telecommunications companies have insisted that there is no scientific evidence that they cause damage to health.

Those defending the masts say the amount of radiation reaching people from a base station is 10,000 times weaker than that produced by an actual cell phone handset.

Ahmad Bin Ali, Etisalat's Corporate Communications Manager, said the GSM base stations or cell phone towers, that the company set up adheres to international conditions and specifications.

He said Etisalat uses guidelines from the Office of Engineering and Technology (OET) in the United States that ensures protection from exposure to radiation.

"Etisalat uses secure limits of maximum exposure when designing and operating phone towers and the locations are selected in line with international safety standards,"he added.
